PLAYER INJURIES DURING A MATCH

TEAM CAPTAINS - Please read all of the information below.

One of the benefits of being an affiliated player is the secondary player medical insurance that is offered through the USASA. To receive the benefits of the secondary player medical insurance, you must properly complete an insurance claim for, sign it, get it sigend by your league representative, then submit the completed original, signed form to the MDCVSA Registrar. All medical insurance claims must be filed through the MDCVSA. Your team captian, coach, or league representative cannot approve an insurance claim, although those persons may be required to sign the claim or provide information in support of the claim. Any injured player should be sure to obtain a copy from the tournament Trainer (if available), signed by a Commonwealth Classic Tournament Committee Member as confirmation that the injury was sustained during the Commonwealth Classic. Include this with the claim submission to the MDCVSA Registrar.
